If you’re on Medicare Part B, you’ll probably get a one-time $90 payment this month. President Trump announced it in a Truth Social post just before 11 p.m. Friday. The White House says more than 20 million people qualify, and most will get it by direct deposit in early October.

Trump’s post called it “nearly $100.” The White House fact sheet says $90. The money comes from the Medicare Improvement Fund, a $2 billion pot Congress created in 2008 that the White House says has never been used.

Who gets the $90 Medicare payment?

Most people enrolled in Medicare Part B. Two groups are left out:

  • People whose Part B premiums are already paid by Medicaid. That covers many lower-income seniors and people with disabilities.
  • Higher-income enrollees who pay IRMAA, the extra income-related monthly charge on top of the standard premium.

You don’t have to apply. If Social Security already has your direct deposit information, the money should land there. Everyone else gets a paper check at the mailing address Medicare has on file.

To check whether you qualify, the White House says to call 1-800-MEDICARE (1-800-633-4227). To check on a payment, call Social Security at 1-800-772-1213. Expect scammers to use this too. Medicare and Social Security won’t call you asking for a bank number to “release” your check.

How much does $90 actually cover?

Less than half of one month. The standard Part B premium in 2026 is $202.90 a month, which is about $2,435 a year. The annual Part B deductible is $283. Medicare’s trustees have projected the premium will rise again in 2027, to about $209.50 a month.

So the payment works out to about 3.7% of what a typical enrollee pays in Part B premiums this year. It’s real money for someone on a fixed income, especially with groceries and gas up. It’s not a change to what Medicare costs.

Why is Trump sending Medicare checks now?

The timing is hard to miss. The payments go out about a month before the November 3 midterm elections. In the same post, Trump said voters “can also rest assured the highly popular $5,000 Trump Dividend will be distributed to every U.S. Citizen if, and when, the Republicans win the Midterm Elections!” That proposed dividend could cost more than $1 trillion and would add to the national debt.

It’s also the latest in a run of direct payments with Trump’s name on them. In September, the administration started sending $500 refund checks, each with a letter from Trump, to nearly 1 million people it says were overcharged for Affordable Care Act coverage.

This money will come from the Medicare Improvement Fund, a pointless “Slush Fund” that has only been used by Dumocrats in Congress to pay for the Waste, Fraud, and Abuse for their Special Interest friends, and drive up Healthcare costs.

His own White House tells the story two ways. The fact sheet says the fund “has never before been utilized” and that no administration has ever made payments from it, while also accusing past Congresses of pulling money out of it for the Affordable Care Act. It’s unclear whether the White House has the legal authority to redirect the money this way, USA Today reported.

Ninety dollars is ninety dollars, and nobody on a fixed income should feel bad about taking it. If you qualify, it’ll show up whether you like the politics or not.

But look at what this is. It’s a one-time payment from a fund Congress set aside to improve Medicare, announced late on a Friday night, about a month before an election, by a president who called it a Democratic slush fund while his own White House says no administration has ever paid out of it. It doesn’t lower premiums, cap drug costs or fix anything about how Medicare works. It’s a rebate with a press release.

The bigger tell is the $5,000 line. Trump says he doesn’t need Congress to pay the dividend, yet it only happens “if, and when” Republicans win. If he believed he had the authority, there’d be no reason to wait. That turns a policy promise into a campaign promise, paid for by money that doesn’t exist yet.

The question worth asking your representative this month: if the Medicare Improvement Fund can be spent this way, what else can a president spend without Congress?
