Tennessee’s Prison Chief Is Resigning After Christa Pike Survived Her Execution

Tennessee’s prison chief is stepping down. Gov. Bill Lee announced Saturday that Department of Correction Commissioner Frank Strada will leave later this month, three days after the state’s failed attempt to execute Christa Pike.

Pike is still alive. Her lawyers say she’s critically ill, unconscious and on a ventilator in a Nashville hospital. Strada’s department had said it followed every step of its execution protocol. Now the man who ran it is out, and a former federal prosecutor is reviewing what went wrong.

Why is Frank Strada resigning?

On Wednesday night, September 30, Tennessee gave Pike two doses of pentobarbital at Riverbend Maximum Security Institution in Nashville. She didn’t die. We covered that night in detail here. She’s believed to be the first person to survive after lethal injection drugs were actually given.

Strada didn’t admit fault in his statement. He said stepping aside was “in the best interest of Tennesseans as the independent review moves forward.” Lee said Strada “has served with integrity” and thanked him for “his willingness to put the interests of Tennesseans first during this difficult moment.” The state hasn’t named an interim commissioner yet.

What do we know now about what happened to Pike?

More than we did Wednesday, and it’s worse. A motion filed by her attorneys, as reported by ABC News, says it took at least seven needles to get IV access, and one came out “bent in a 90-degree angle.” Witnesses heard her crying and breathing loudly, and she said her arm felt “like it’s about to burst open.” Her lawyers describe her arms as “swollen, burned, and blistered.”

Before the execution, her legal team had argued exactly this risk. They said she had small veins and a blood condition that would make placing an IV difficult. Tennessee’s courts rejected the claim, and the U.S. Supreme Court let the execution go forward.

Is this the first problem on Strada’s watch?

No. In May, Tennessee called off the execution of Tony Carruthers after the team couldn’t get IV lines in, despite repeated tries in different parts of his body. That’s two failures in five months in the same chamber.

Before Tennessee hired him in January 2023, Strada was a deputy director of Arizona’s prison system. Three Arizona executions in 2022 happened on his watch, and the Death Penalty Information Center called them visibly problematic. Pike’s lawyers say those were “marked by prolonged and problematic attempts to establish IV access,” which is the same problem Tennessee just had twice. Arizona’s new governor, Katie Hobbs, ordered an independent review and paused executions in 2023, the same year Strada left for Tennessee.

What happens next?

  • Former U.S. Attorney Ed Stanton will lead an independent review of whether the protocol was followed and how to fix it. He also reviewed Tennessee’s lethal injection process in 2022.
  • Lee has suspended the executions still scheduled for this year until the review is done.
  • Pike remains on death row. Whether Tennessee can try to execute her again is an open legal question with almost no modern precedent.

Pike’s attorneys put it bluntly: “The resignation of Commissioner Strada is justified, but it does nothing to help Christa now.”

Strada’s resignation is the right call, and it’s also the easy one. One official leaving lets everyone else avoid the bigger question.

The record was there before Pike was strapped down. Tennessee hired a man whose Arizona executions had IV trouble. It watched the same failure with Carruthers in May. Pike’s lawyers told the courts, in writing, that her veins would cause exactly this. Every person who could have stopped it had that information and went ahead. That’s not one commissioner’s mistake.

Stanton’s review needs to answer whether Tennessee’s process can work at all, not just whether staff followed it. The department already said they followed it. If that’s true, the protocol is the problem, and it’ll still be there after Strada’s gone.

The question nobody in Nashville wants to answer: if Pike recovers, will Tennessee try again?
